Must have a qualifying condition, have an existing provider/patient relationship and must submit all medical records with qualifying diagnosis as well as a state photo ID at your visit. Once you make your appointment, you will be sent a letter of instruction on qualifying conditions as outlined in the Utah State Medical Cannabis Act.
